2-Year Interim Report

The Commission on Independent Schools requires that you submit a Two-Year Interim Evaluation Report due on the date noted in the letter received by the school following Commission action on the evaluation visit.  Schools will receive a reminder letter with forms enclosed at least 6 months prior to the report due date.  Please submit two copies of the report and all supporting documents.

If you are jointly accredited by NEASC and the Association of Christian Schools International (ACSI), please use this form:

Otherwise, select the report form that coincides with the edition of the Manual for School Evaluation used by your school to prepare the self-study.  Most schools will use the 2005 Pilot/2007 Editions of the form.
