Annual Report Form
Every school accredited by the Commission on Independent Schools is required to submit an Annual Report. Forms are sent to schools by early October and are due in the Commission office by November 30th each year.
Annual Reports can be completed on line after October 1st of the current school year. Please click here to complete the form.
